Cats can recognize tones and emotions in our voices, which is why they understand us. They can also distinguish voices from others, meaning they can tell the difference between your voice and someone else's.
As for whether or not your pet thinks its your parent, well, that all depends on the age your cat was when you got them. If cats are taken away from their mother too young, they will develop a neurotic bond with their new owner, meaning they basically replace their mother with you.
This sounds great, but it is actually not healthy for the cats development. Regardless, your cat will know that you are not its actual birth parent, obviously, but will probably see you in the same way.
As a pet owner, you protect, love, care for, and provide for you pet, which is essentially what a mother does for her kitten. They will know the difference between you and their mother, but will still see you as a supreme being in one way or another.
